♦ School Recommendation Form (Pre-K-8th Grade)
♦ Release of Information Form (Pre-K-8th Grade)
♦ Photo of Student
♦ Copy of Birth Certificate
♦ Copy of Baptismal Certificate (if Catholic)
♦ Copy of Report Cards for prior 2 years (1st-8th Grade)
♦ Copy of Standardized Test Scores for prior 2 years (3rd-8th Grade)
♦ Copy of IEP (Individual Education Plan), if applicable
♦ Pre-K-Kindergarten: Play Session in Classroom
♦ 1st-8th Grade: Interview and/or Buddy Day
