- Non-Refundable Application Fee ($100.00)
- Writing Sample (3rd-8th Grade) – Handwritten
- School Recommendation Form (Pre-K-8th Grade)
- Release of Information Form (Pre-K-8th Grade)
- Photo of Student
- Copy of Birth Certificate
- Copy of Baptismal Certificate (if Catholic)
- Copy of Report Cards for prior 2 years (1st-8th Grade)
- Copy of Standardized Test Scores for prior 2 years (3rd-8th Grade)
- Copy of IEP (Individual Education Plan), if applicable
- Pre-K-Kindergarten: Play Session in Classroom
- 1st-8th Grade: Interview and/or Buddy Day
